Site Safety Observation

Instructions:

Use the below points to guide in identifying unsafe acts or conditions in need of correction. Conduct twice per shift (at minimum)

General Site Safety:

  • All equipment operators are trained & authorized.
  • Fall Protection and PPE are in use as required (fall protection gear, hard hat, steel toes, high vis).
  • Tool & equipment pre-use/shift inspections are completed by operator(s) (eg. forklift, cranes, fall protection).
  • Seatbelts are worn at all times while mobile equipment is operational *if equipped.
  • Defective tooling / equipment is removed from service, tagged & locked out.
  • Tool & equipment guards are intact; not bypassed, removed or tampered with.
  • Vehicles & mobile equipment are operating in safe manner (eg. used as intended, at safe speeds)
  • Work areas are organized & clear of observable hazards (eg. slip / trip hazards, unstable stacks, blocked exits, poor housekeeping, etc.)

Include the details of what happened before, during, and after the event. Describe who was involved, the job being performed, and where the incident took place.
